Physicians Realty Trust (NYSE: DOC) Loss Submission Form

Kuznicki Law is investigating the proposed sale of  Physicians Realty Trust (NYSE: DOC) to Healthpeak Properties, Inc. (NYSE: PEAK).  Under the terms of the proposed transaction, shareholders of Physicians Realty will receive 0.674 of a newly issued Healthpeak common share for each share of Physicians Realty that they own.
